By submitting this water application, I am requesting the City of Monrovia to supply water service to the service address submitted above. I understand that my information may be used by other City departments for official City business only and will not be shared or used for any other purpose. I will comply with City ordinances, rules, and regulations and agree to pay any fines or fees imposed for failing to do so. I have the legal right to request service and I acknowledge that I have read and understand this application. A $25.00 NON-REFUNDABLE WATER SERVICE ACTIVATION FEE WILL BE ADDED TO YOUR FIRST WATER BILL.

PLEASE MAKE SURE THAT ALL WATER VALVES SUCH AS FAUCETS, HOSE BIBS, TOILETS, ICE MAKERS, ETC. ARE TURNED OFF. OUR TECHNICIANS CANNOT TURN THE WATER ON IF THEY NOTICE THE WATER IS RUNNING.
